Adult Function Report option Function Report, SSA 3373, is an eleven page form used to obtain the information about how your disability affects your ability to function with daily activities. The claims examiner uses this information to assess the “alleged” disability. The information, together with medical evidence, forms the evidentiary basis upon which the initial disability evaluation process is founded. To be effective this form must be completed in detail. Although this is an optional form to submit with your application, it is highly recommended. Many claims examiners will request you to complete so, it’s better to complete now than delay the process of getting your benefits. View the PDF file of the Adult Function Report.
With the Adult Function Report Option SSDI-Services will:
1. Prepare electronic files with personal data.
2. Conduct an in-depth phone interview
3. Write descriptive answers with objective measurement
4. Input answers on the form
5. Submit the completed form with the application package
